Compensation range is $17 to $29 per hour.  Compensation is dependent upon geographic region, education, and experience

Job Description

The Case Manager is responsible for performing patient care activities as directed by the program manager, orienting, supporting discharge planning from intake to discharge, building rapport with referral sources, and connecting clients to community resources. The Case Manager’s responsibility is to provide direct support and resources to the client and clinical staff.  This position is 100% remote. 

Responsibilities

  • Support Discharge Planning nationwide with Virtual IOP Programming (CA, CT, TX, MD, FL)
  • Actively participates in Multidisciplinary Treatment Team meetings
  • Serves as the liaison between the clinical and Outreach department  
  • Facilitates psychoeducational groups at the directive of the program manager 
  • Shares pertinent information with team as obtained from client’s family, or others as indicated throughout the program.  
  • Recognizes, responds to and assists in psychiatric crisis situations and provide the intervention necessary
  • Develop resources to provide clinical and clients in order to meet client needs after treatment
  • Communicates effectively with client and interdisciplinary team regarding client needs 
  • Assists with chart documentation and compliance

Qualifications

 

  • Graduate from an accredited college or university with a minimum of Bachelor's Degree in Psychology, Social Work, Counseling, or health-related field
  • CEU certifications and on-going training in health-related field
  • Knowledge of Excel, Zoom, and comfortability with technology
  • Knowledge of the Principles and Practices of the discipline
  • Knowledge of Joint Commission Standards
