Compensation Range: $75-95k
Compensation will be dependent upon geographic region, education, and experience.

The Director of Care Coordination is responsible for ensuring that the interactions and experiences patients, loved ones, and referral partners have with Discovery Behavioral Health are exemplary and consistent, regardless of brand, services, or level of care. This is accomplished by collaborating with facility-based employees, clinical outreach, strategic partners, and patient support system points of contact. This director will manage and supervise onsite case managers (and/or care coordinators), and report to the Care Coordination department with additional reporting responsibilities to the facility leadership. The Director of Care Coordination also focuses on maintaining and improving strategic referral partners by ensuring excellent communication with our strategic referent relationships. Ideal candidate has experience in management, working at a facility as part of a treatment team, and in business development.
This is accomplished by training and collaborating with all DBH departments including operations, admissions, payer relations, marketing, and clinical outreach. Additional responsibilities include conducting patient and referral satisfaction studies and reporting results.
Essential Job Functions:
- Management of onsite case management/care coordination team. Ensuring case managers/care coordinators are regularly meeting with patients for discharge planning, transition plans, and treatment support. Ensuring case managers are meeting KPIs as determined by facility and outreach leadership.
- Attends weekly and appropriate clinical meetings for client updates and discharge planning discussions.
- Primary contact between DBH facility, clinical outreach, and referral sources in assuring communication and client care coordination is happening in a timely and appropriate manner.
- Train and oversee clinical team in support of outreach corporate initiatives regarding client aftercare coordination with our preferred providers. This includes but is not limited to documentation in Kipu, Salesforce, and other areas as deemed appropriate.
- Ensures referral source communication occurs timely with patients in our care and those partners associated with aftercare planning with timely documentation.
- Identify opportunities to refer patients in our care to preferred and strategic aftercare partners based on clinical outreach recommendations. Secure appropriate ROIs, VOBs, assessments and seamless transition for step down to SL, IOPs and therapists/psychiatrists.
- Ensure when appropriate that we are referring patients within the DBH system for continued care.
- Coordinate tours with clinical outreach and external referral sources ensuring visitors are experiencing an excellent and consistent experience at DBH.
- Facilitate aftercare groups and individual meetings with clients regarding discharge planning.
- Work closely with admissions team to identify strategic opportunities to refer patients to other divisions within DBH and other strategic referral partners when not appropriate for Discovery.
- Collaborate with family programming to provide resources to families.
- Collaborate with alumni team to identify any opportunities to provide resources and referrals to our treatment partners.
- Minimum of 4 years’ experience in a mental health and/or chemical dependency setting is preferred
- Experience working with clients and documentation in a hospital and/or mental health facility required
- Strong communication, leadership, and collaboration skills.
- Knowledge or ability to learn Salesforce, KIPU or other EMR system
