Compensation Range: $25.00-34.00 per hour

Compensation will be dependent upon geographic region, education, and experience.

Job Description

The Case Manager is responsible for strategizing and planning with their clients an affective aftercare plan, as well as building a strong relapse prevention plan with clients during treatment with Cliffside Malibu. The Case Manager fulfills direct service, including individual and group psychotherapy, family contact, consultations with treatment team and crisis interventions. Case Manager is responsible for a full case load and timely charting in the Electronic Health Record. 

Responsibilities

  • Knowledge of substance use disorders, mental illness, family systems, and recovery resources in the community. Knowledge of current ethical and legal standards pertaining to professional practice, reporting requirements, professional boundaries and related issues.
  • Knowledge of company’s policy and procedures, DHCS, Joint Commission standards and regulations and Insurance Clinical Guidelines. Adhere to all regulations and documentation requirements in regards to client care and compliance of all client care. Meet compliance deadlines.
  • Knowledge of group facilitation and responsible for running groups at Cliffside Malibu organizations.
  • Knowledge of The Stages of Change and the Clinical philosophy of Cliffside Malibu.
  • Develop and maintain individualized records of participants progress and participation in treatment including admission and discharge, individual, group, family, vocational (both individual and group) as appropriate.
  • Ability to write clean and concise statements, summarizing participant’s feelings, expressions, resistance and response to their treatment plan; goals and objectives.
  • Ability to respond to client requests for letters to referral agencies, courts/probation, gather information and provide to Clinical/Program Director.
  • Ability to confront behavioral changes, identify a crisis and recognize when additional assistance is necessary.
  • Assumes responsibility of knowing when clients are in need of extensions or referrals, and actively participates in obtaining appropriate extensions/referral. Ensure appropriate Coordination of Care is facilitated. Ensure family and referent are contacted throughout client’s treatment episode. Provide accurate documentation to the Utilization Review Department of Cliffside Malibu Organization. Ensure referrals are made to (i) outside therapist/psychiatrists, Doctors etc. and (ii) to appropriate levels of care. Confirm appropriate medical care/medications are being provided to client. Approve ancillary appointments, and verify they are being kept. Work directly with Primary Therapist, MD and Psychiatrists. Ensure the milieu is therapeutic and physically safe at all times.
  • Ability to demonstrate respect for cultural and lifestyle diversities of participants and staff.
  • Ability to prepare for and facilitate groups, trainings and presentations.
  • Participate in case conference reviews. Attend weekly clinical/treatment team meeting. Keep an open line of communication regarding client’s condition, appointments, requests and approvals, with Clinical Manager, Executive Director, medical and office staff.
  • Ability to ensure clients meet program requirements for authorized level of care.
  • Ability to inform the treatment team and utilization review of clients discharge from program.

This is a full-time position that will work 4 days / 10 hour shifts. The schedule needed is:

 

  • Sunday 9am to 7:30pm
  • Monday 9am to 7:30pm
  • Wednesday 9am to 7:30pm
  • Thursday 9am to 7:30pm

Qualifications

  • California Certified Alcohol & Drug Counselor
  • Recommended minimum High-school diploma or equivalent
  • Must be Certified
  • Significant experience working in substance use and mental health treatment. This includes knowledge of and experience in delivering all modalities of treatment, group and individual.
  • Valid Class “C” Driver’s License and liability insurance.
  • Vision, hearing, manual dexterity and hand-eye coordination; must be adequate for performance e.g. able to sit at a desk, safe driver, use computer, write clear and concise statements, move about the facility
  • TB test prior to hire and current thereafter
  • Clean U/A test.
  • First aid/CPR certification, obtain within 90 days and maintain current thereafter
  • 1-year minimum sobriety, if in recovery
  • Must demonstrate the ability to write competently. This includes case evaluations, exit plans, utilization reviews, and recommendations.
  • Understanding of Electronic Medical Records
