Compensation Range: $19-26/hr.
Compensation will be dependent upon geographic region, education, and experience

Care Coordinators are responsible for ensuring a seamless transition of patient care between programs. This includes, but is not limited to, ensuring that the interactions and experiences of patients, loved ones, and referral partners have with Discovery Behavioral Health are exemplary and consistent, regardless of brand, services, or level of care. This is accomplished by ensuring all points of contact within the DBH system are timely, meaningful, and exceptional, from inquiry through discharge and aftercare. Care coordinators are also responsible for identifying opportunities to transition patients within the DBH family of brands and services as appropriate to ensure they have the best possible chance for recovery.
Care Coordinators connect with callers, patients, and D365 users regularly to assess treatment needs and work closely with admissions and clinical outreach to provide appropriate referrals for follow up care. This is accomplished through coordination with patient’s treatment teams, assigned admissions coordinator, clinical outreach, care coordination teams, and other appropriate staff to ensure quality and appropriate care is recommended and planned.
Essential Job Functions:
- Identifies callers, patients, and D365 users appropriate for DBH programs through collaboration with admissions and utilization of Kipu and Salesforce systems.
- Supports DBH clinical and care coordination teams in discharge planning by identifying DBH or outside resources as appropriate for continuation of patient care.
- Supports in the development of discharge plans that are in alignment with CSQM goals, treatment team recommendations, clinical outreach, and patient’s individual needs.
- Communicates with patient treatment team, internally and externally, and other appropriate parties as indicated ensuring HIPPA compliance.
- Attends Treatment Team meetings as appropriate to ensure clinical collaboration and patient care coordination. Attendance of additional meetings may be required as deemed appropriate by facility and Care Coordination leadership.
- Completes documentation for patient and caller interaction as appropriate in DBH EMR and CRM.
- Documents outbound, discharge, and D365 referrals in the patient’s medical and salesforce records as appropriate.
- Thorough research of recommended care options including outpatient services, Sober Livings, therapists, psychiatrists, etc. to ensure quality referrals. Research conducted will include primarily clinical outreach recommended referrals, internet screening for well-trusted services, phone calls to verify appropriateness and reserving available space if transitioning to a sober living.
- Performs other duties and tasks as assigned.
- Experience working with clients and documentation in a hospital and/or mental health facility preferred.
- Required to possess one of the following:
- Bachelor’s Degree
- CDAC (Certified Alcohol and Drug Counselor) Certificate.
