Compensation Range: $50,000 - 57,000/annually
Compensation will be dependent upon geographic region, education, and experience.

The Prosperity Wellness Center SUDP/Assessor is responsible for assessments, intakes, educational groups, and individual sessions. Additionally, the Chemical Dependency Professional must possess skills in crisis interventions, patient motivational skills to engage, as well as patient discharge confrontations when necessary.
Responsibilities
- Provides role-modeling behavior.
- Lead agency assessor
- Other responsibilities as assigned include intakes, treatment plan development, file reviews, progress notes, aftercare plans, and discharge summaries.
- Responsible for facilitating process, educational groups, and individual session.
- Must engage in crisis interventions and patient motivational skills, as well as patient discharge confrontations when necessary.
- Responsible for timely and proficient case management and documentation to include ensuring all necessary paperwork is sent in a timely manner for admission and discharge.
- Perform other duties as assigned by supervisor.
This is a full-time, salary position that will work Monday through Friday from 8:00am to 4:30pm.
- Must have an active SUDP or SUDPT credential--with no enforcements.
- Bachelor’s degree in health care, social work, psychology, or another related field
- Must be proficient in typing and managing multiple assessments.
- Completed 2,500 hours of supervised experience within the field of substance abuse or another related field.
- Prior experience and in-depth knowledge of SUD assessment
- Prior experience utilizing motivational interviewing, group facilitation, and case management.
